Get a picture ? は Do you get the picture ? を簡略した言い方です。動詞 get には「～を理解する」という意味があり、この場合の pictureは「写真」ではなく「状況／事態／全体像」などの意味で使っています。「全体像（絵）をつかめましたか？」つまり、相手に状況を説明して「全体の流れを理解できましか？＝だいたい分かりましたか？」と尋ねるときのカジュアルな定番のフレーズです。自分自身が理解してたと相手に伝えるときには I get the picture. と言います。似た表現に get the message. もあります。
